I am in network with United Health and Cigna.

I am in network with the following BCBS plans: all PPO plans, blue essentials HMO, blue advantage HMO, blue premier HMO, blue high performance, my blue health HMO,  medicare advantage PPO and HMO.

I am in network with the following Aetna plans: PPO, POS, EPO, HMO, and medicare. 

 

Please be aware that some HMO plans require a referral from a primary care provider. It is the patient's responsibility to verify if their plan requires a referral.

If I am not in your network, I will gladly provide a superbill so your payments can be applied towards your out-of-network benefits.
